Affiliation forms are for those teams going to Provincials (or National Eliminations) that wish to affiliate with a team outside of their association (Example: Cape Breton 18U AA wishes to affiliate with Antigonish 18U A)
Affiliation forms are not necessary if you are affiliating with a team directly below you in your association (Example: Hammonds Plains 18U AAA is not required to complete a form to affiliate with Hammonds Plains 15U AAA)
